If your name differs from that reflected on your documents for submission OR to change your account name: Submit proof of legal name change. Acceptable forms of evidence include LEGIBLE copies of the following in which BOTH names must be reflected: marriage or divorce decree; valid (non-expired) driver's license or passport, legal name change decree, or HR attestation of review of legal name change documentation.
